Most of the kitchens we do start with the cabinets. Painting the boxes you have and fitting new doors and hardware is the fast version; new cabinets, a new layout and an island is the full one. Either way the counters, the backsplash and the floor are done in the same job, so the room is finished once rather than in stages.
Opening a kitchen to the living room means moving a wall, and moving a wall means the electrical and plumbing behind it move too. Our electricians and plumbers are licensed and on our own crew, so that work runs on our schedule rather than around someone else's.
